Hormone Replacement Therapy for Women

Hormone Replacement Therapy for Women

Bioidentical hormone optimization for perimenopause, menopause, and beyond — estrogen, progesterone, testosterone, and DHEA.

RESTOREHormones & Metabolism

By the time most women reach their mid-40s, their hormones have already been shifting for years. Perimenopause doesn’t announce itself with a single event — it shows up as worsening sleep, unexplained weight gain around the midsection, brain fog that makes you feel like you’re losing your edge, anxiety that appeared out of nowhere, and a libido that quietly disappeared. Your doctor runs a basic panel, tells you your levels are “normal for your age,” and suggests an antidepressant. That’s not medicine. That’s resignation.

At Longitude Life, we don’t accept “normal for your age” as a clinical endpoint. Your DECODE workup maps the full hormonal architecture — estradiol, progesterone, total and free testosterone, DHEA-S, pregnenolone, SHBG, thyroid cascade (TSH, free T3, free T4, reverse T3), cortisol rhythm, and insulin sensitivity. We’re not checking a box. We’re building a complete picture of how your endocrine system is actually functioning so your protocol is built on data, not assumptions.

How We Treat

Bioidentical hormone replacement is the foundation. We use transdermal estradiol (patches or cream) rather than oral estrogen — it bypasses first-pass liver metabolism, avoids clotting risk elevation, and produces steadier serum levels. Micronized progesterone (oral, typically at bedtime) protects the endometrium, supports sleep architecture, and has anxiolytic properties that synthetic progestins lack entirely. For women with declining libido, energy, and muscle tone, we add testosterone cream at physiologic female doses — a tool most conventional gynecologists never even consider. DHEA supplementation rounds out the protocol when adrenal output has declined.

What Makes This Different

Every protocol is reassessed at 90 days with repeat labs. We’re tracking not just your hormone levels but your metabolic response — body composition via InBody, inflammatory markers, lipids, and how you actually feel. Your protocol evolves as your body changes through perimenopause, menopause, and beyond. This is the SUSTAIN pillar in practice: ongoing monitoring, ongoing adjustment, ongoing optimization.

Both the Precision Protocol ($499/month with $200 medication credit) and the Longitude Protocol ($899/month with $300 medication credit) include comprehensive hormone management. Most single-hormone protocols fall within the medication credit — there’s nothing extra to pay.
